  • Make it with your parent, not about them. Ask what they want shared, and with whom.
  • Write down logistics, not medical details. Note that an appointment exists, not what it’s for.
  • Never write passwords, PINs or account numbers on these pages.
  • Agree who can see it, and where a printed copy is kept.

Call a longer family conversation when

  • something big changes in your parent’s home or routine;
  • someone can’t keep up their share any more;
  • a decision needs everyone, and your parent, in the room.

Doing it by group chat

Post the three questions on the same day each week. Everyone answers in one message; whoever runs it that week posts the decision and the actions.

Share with care. A worksheet link opens a blank copy. To share your completed plan, review it and print or copy the parts you want to share. Ask your parent what they want shared, and with whom.

Summary to share

Read it through and change anything you like. Nothing is sent: you choose where to paste it. Private notes are never included.
